Full Series Withdrawal Form

This form is to establish the END DATE for an ongoing enrollment Series of a lesson spot. A minimum 14 day notice is required to permanently remove your child from their lesson. In the case of a medical situation, a doctors note must be submitted for an exception. If your account balance is not settled before your child’s last lesson you will incur a $25 cancellation fee per lesson spot. Please confirm all cancellations and schedule changes with our office and not the instructors.
